Emergency Contact Form
If you need to update your child’s emergency contact information, please complete the Emergency Contact Change Form. This form, along with a copy of the enrolling parent or guardian’s photo ID, must be submitted if someone not currently listed in our system will be picking up your child.
For student safety, only individuals identified as approved emergency contacts will be allowed to pick up students from school.
